CDX2: prognostic marker for high-risk colon cancer

Over the past decade, steady improvements in disease-free survival (DFS) outcomes have been observed in patients with stage III colon cancer, owing to the availability of new adjuvant treatment regimens. However, no reliable single biomarker exists that enables stratification of patients with high-risk, stage II disease for appropriate therapy. Patients with stage II colon cancer are often cured with surgery alone, but 15–20% of patients relapse and eventually succumb to their disease. Thus, understanding which patients with stage II colon cancer could benefit from treatment with adjuvant chemotherapy is a research priority.

The use of multigene-expression signatures that are prognostic of disease relapse has not been easy to apply in the clinic. Now, Piero Dalerba, Debashis Sahoo, Michael Clarke and co-authors show that a lack of CDX2 expression can be used to identify patients with stage II colon cancer who are at high risk of disease progression and who would benefit from adjuvant chemotherapy.
