  • A randomized multicentre controlled study of 240 cardiac surgery patients at high risk of acute kidney injury (AKI) has demonstrated that remote ischaemic preconditioning can reduce the rate of AKI and requirement for renal replacement therapy. These findings suggest this procedure could be a promising therapeutic option for this high-risk patient group.

  • Mitral valve disease is a common cause of heart failure and death. In this Review from members of the Leducq Transatlantic Network, Levine and colleagues extensively summarize the molecular and cellular mechanisms underlying the dynamic changes that occur during progression of mitral valve disease, and indicate how improved understanding of the pathophysiology might lead to the discovery of novel therapeutic options.

  • Acute kidney injury (AKI) is associated with high levels of mortality and an increased risk of progressive chronic kidney disease. Here, David Ferenbach and Joseph Bonventre describe the mechanisms involved in the initiation and propagation of AKI. They discuss the prototypic mechanisms employed by the damaged kidney to repair, and how the adaptive processes of repair can become maladaptive, leading to the development of progressive fibrotic kidney disease and a state that can be considered accelerated kidney ageing.

  • Fetal exposure to an adverse intrauterine environment is associated with an increased risk of numerous chronic diseases in later life. Here, Luyckx and Brenner highlight the emerging developmental risk factors for chronic kidney disease and hypertension, including high birth weight, maternal diabetes mellitus and rapid growth during childhood. The authors discuss the burden imposed on resource-poor countries by suboptimal nutritional intake, and address the benefits of nutritional interventions in reducing the global epidemic of these chronic diseases.
