  • The Acute Kidney Injury Network (AKIN) met in Amsterdam, The Netherlands, in September 2005. At the 2-day meeting, participants in this multidisciplinary collaboration proposed a standard definition, and scheme for the classification, of AKI. Widespread adoption of these standards, presented here by members of the AKIN working group, would help to advance the study of this condition, and improve patient outcomes.

  • Blood pressure control is influenced by amiloride-sensitive sodium channels in the vascular and epithelial homeostatic systems in the kidney. Here, the authors describe the expression and regulation of sodium channels, and they outline the emerging evidence that differences between sodium channel complexes expressed in the epithelia and endothelia might permit novel therapeutic approaches to lower systemic blood pressure without the adverse effects associated with the sodium channel blockers that are currently available.

Michael et al. review the current safety data for the nondextran intravenous iron formulation sodium ferric gluconate complex, and examine the emerging concerns related to use of intravenous iron, including the risks of infection, oxidative stress and cell toxicity.

  • In this Review, participants in the 7thInternational Fabry Nephropathy Roundtable, which was convened in Barcelona, Spain in June 2007, propose guidelines and recommendations for the diagnosis and management of Fabry nephropathy in adults. The roles of enzyme replacement and antiproteinuric therapy are emphasized. The authors suggest that these organ-specific recommendations could have a greater impact on clinical practice than general Fabry disease guidelines.

  • Estimates of the prevalence of chronic kidney disease (CKD) vary widely, both within and between countries. Here, the authors discuss the origins of this variation, particularly issues relating to the use of estimated glomerular filtration rate, and present solutions for tackling the factors responsible.
